Best Fertilizer for Carrots

No one does soil testing in our little northern town. I grow only carrots in raised rectangular beds (3 X 10)ft. Last year I had a stunted crop of carrots that were nowhere near the size that they should have been. I know now that this was due to a lack of fertilizer. Could you tell me what formulation of fertilizer is the best for my needs and how much of it would I have to work into the soil?

You might want to enhance the soil with compost and I've had great luck with soil from under an old burn pile (logging). So let's assume I get a terrific crop of carrots this season after having used the carrot fertilizer.Does that mean that the carrot fertilizer is depleted and I should be applying the same amount of fertilizer next year?

That's an interesting question since nitrogen is depleted quickly but phosphorus can stay in the soil for years.
