Check with your local extension office. They will have soil testing kits available and be able to give information on how to properly set it up. Most times the fees are quite inexpensive.

Presque Isle, MI(Zone 5a)

I test my soil every few years. If your plants are not doing what you expect, do a soil test. In Michigan it cost me $26.00 for two different samples.

Send an email to the people at this site http://algreatlakes.com/lab_hom_law.asp to see if they do your area. I'm guessing they do. I mailed them a sample, they have very excellent service, and I got back a very nice report. Only $15.

UConn charges $8 for one. Didn't see anything about tests being confined to soil in Connecticut. Here is mine from this spring (they give recommendations. too but I didn't scan that part):
