Have a 25 yo yellow azalea. I moved it a year ago from sandy loam to amended clay. It had several flower scapes last spring...only 1 viable branch this year!!!!
My sister got it from Walnolong nursery formerly in Morris Plains NJ
It is an old variety.

I think that nursery closed when or after the owners passed away. Have you tried emailing Rarefind, Weston or Greer Gardens Nurseries? Rarefind is in NJ. Weston Nurseries is somewhere in the southeast coast of FL. Greer Gardens is in Oregon. I would include pictures with blooms. There is a book that you can also research which may help; I am probably going to get it wrong but the title may have been something like Color Book of Rhododendrons and Azaleas. I do not remember who the author is.

The American Rhododendron Society has a plant database available online. There is one page for searching azaleas and another for searching rhododendrons. You can key on flower color (or other parameters) and the choices presented sometimes have pictures. See this link:

Azaleas do not do well in clayish soil. I have to use raised beds instead of a soil mix because of clay.

It looks like a Deciduous az Hybrid. There are many of these in the trade. The ones from Minnissota University are probably the hardiest ones. Look in the mail order catalogs, for them. Rare Find Nursery in Jackson NJ is a good source.
My "round to it" habit may have worked...I saw tiny leaves budding (size of a small ant) ....usually this habit isn't a good thing!!!!!!!!!!!!!!!!!!!!! but just maybe !!!
