Who is making these large holes?

Any ideas on who could be making these 2+ inch holes?

For reference, on the upper right is a viola, bottom is a maple seedling. This is below a black lace elderberry, whose leaves on the ground.

It is usually moles and/or voles. They would be the same reason last year's tulips don't come up next year.
