How do you make your labels for your quilts?

I guess I've just never looked at the backs of quilts to see how these are done. I used to order my name labels to put in clothes and such when I sewed years ago, but now that I am into quilting, I've noticed where people speak of attaching labels on the backs for dates made, etc.
Do you design these from software, embroider by hand, order them....??? Or do you use the treated fabric in the printer and print it that way, like you would address labels? Now that I've almost got my daughter in law's quilt finished, I thought it would be nice to do this, only now I'm stumped...
Is there a link to some ideas or designs?
